Each episode features a detailed dramatized reconstruction of the incident based on cockpit voice-recorders and air-traffic-control transcripts, as well as eyewitness accounts, interviews with aviation experts, including interviews with investigators who dealt with the disaster, and sometimes even actual footage.

Slam DunkOn 7 January 1994, United Express Flight 6291, operating for Atlantic Coast Airlines, crashes 1.2 miles short of the runway during final approach to Port Columbus International Airport after entering a stall. The two pilots, the flight attendant, and two passengers are killed in the crash.
